The Slow Pandemic

HIV crossed from chimpanzees to humans early in the twentieth century, and genetics has mapped its rise: from the 1920s, Kinshasa was the hub where the pandemic strain established itself and spread along Central Africa's railways, decades before the first recognized cases of 1981. The virus has since infected over eighty million people and killed tens of millions; antiretroviral therapy turned infection from a death sentence into a managed condition, though neither cure nor vaccine yet exists.

Why it matters

The deadliest new pathogen of the modern era ran silent for sixty years, which is the standing argument for viral surveillance at the animal frontier. Its history also rewrote the story we tell about pandemics: no patient zero, but a city, a transport network and colonial-era social change giving a chimpanzee virus its chance. And the therapy that contains it is molecular biology's most consequential victory since antibiotics.

Dating & uncertainty

The syndrome was recognized in 1981 and the virus soon after; genetics then pushed the true beginning back sixty years. Phylogeography of viral sequences, validated against the oldest archival sample, places the pandemic strain's early spread in 1920s Kinshasa, radiating along the Congo basin's railways and rivers, with an epidemiological takeoff after 1960. The pandemic was half a century old before it had a name.
