Clean Water

In 1854, mapping cholera deaths around a single London water pump, John Snow demonstrated that the disease travelled by water, not air, and had the pump handle removed: epidemiology's founding act, a generation before the germ itself was seen. When cities then filtered and chlorinated their supplies, the effect was enormous and is now measured: clean water explains nearly half of the total urban mortality decline in early twentieth-century America, and three quarters of the fall in infant deaths.

Why it matters

No drug has saved as many lives as plumbing. Clean water broke the ancient death grip of cities, which until then buried more people than they raised, and it did so before medicine could cure anything at all. The task is unfinished: inadequate water, sanitation and hygiene still cause on the order of 800,000 diarrhoeal deaths a year, most of them children, a nineteenth-century problem persisting inside the twenty-first.

The anchor is John Snow's 1854 investigation of the Broad Street cholera outbreak, epidemiology's founding case. The measurable revolution came with filtration and chlorination decades later: in American cities, clean water technologies account for nearly half of the total mortality decline of the early twentieth century, and three quarters of the fall in infant deaths.
